چکیده

Cardiac resynchronization therapy (CRT) is a cornerstone of treatment for advanced heart failure, but not all patients derive equal benefit from this intervention. Current approaches using electrocardiographic and echocardiographic parameters to identify likely CRT responders demonstrate poor predictive ability. We seek that can improve prediction with This was retrospective analysis who underwent CRT-D placement at University Pittsburgh Medical Center 2014-2020. Patients were excluded if pre-implantation EKG or pre- post-implantation echocardiograms available. Each patient’s immediate ECG processed the CineECG software (EKG Excellence) obtain mean temporospatial isochrone ventricular activation. 192 features quantifying activation recovery pathways computed each patient. Response defined as an improvement in ejection fraction (EF) 5% on follow-up transthoracic echocardiogram 3-12 months after implantation. Forward sequential feature selection used highest importance based negative logistic loss. 276 included (36% female, age 69 years, pre-procedural EF 27%). The 10 most important identified. When these multivariate regression, three independently response: left bundle branch block (LBBB); leftward direction late repolarization potentials (i.e., Tpeak - Tend rotated toward LV horizontal plane); location center-of-mass terminal cardiac Compared (n=169, 62%), non-responders more have abnormal both LBBB group (-0.34±0.30 vs. -0.45±0.30 vs, p=0.027) RBBB (0.25±0.51 0.61±0.27, p=0.031). In latter group, later correlated magnitude change post-procedural (r=0.23, p=0.016). Our findings confirm presence response CRT. addition, our data suggests RBBB, response. Further studies into dynamics predictor are warranted.

Dispersion of repolarization in cardiac resynchronization therapy.

BACKGROUND Proarrhythmic effects of cardiac resynchronization therapy (CRT) as a result of increased transmural dispersion of repolarization (TDR) induced by left ventricular (LV) epicardial pacing in a subset of vulnerable patients have been reported. The possibility of identifying these patients by ECG repolarization indices has been suggested. OBJECTIVES The purpose of this study was to te...

ECG Patterns In Cardiac Resynchronization Therapy.

Cardiac resynchronization therapy is an established treatment modality in heart failure. Though non-response is a serious issue. To address this issue, a good understanding of the electrical activation during underlying intrinsic ventricular activation, biventricular as well as right- and left ventricular pacing is needed. By interpreting the 12-lead electrocardiogram, possible reasons for subo...

A new baseline scoring system may help to predict response to cardiac resynchronization therapy

INTRODUCTION The PROSPECT trial reported no single echocardiographic measurement of dyssynchrony is recommended to improve patient selection for cardiac resynchronization therapy (CRT). MATERIAL AND METHODS In 100 consecutive patients who received CRT, we analyzed 27 ECG and echocardiographic variables to predict a positive response to CRT defined as a left ventricular (LV) end systolic volum...

What is the Lowest Value of Left Ventricular Baseline Ejection Fraction that Predicts Response to Cardiac Resynchronization Therapy?

BACKGROUND Cardiac resynchronization therapy (CRT) is an effective treatment option for patients with refractory heart failure. However, many patients do not respond to therapy. Although it has been thought that there was no relation between response to CRT and baseline ejection fraction (EF), the response rate of patients with different baseline LVEF to CRT has not been evaluated in severe lef...

Cardiac sympathetic reserve and response to cardiac resynchronization therapy.

BACKGROUND The objective of the present study was to investigate the effect of cardiac resynchronization therapy (CRT) on cardiac autonomic function. METHODS AND RESULTS This prospective study included 45 consecutive patients with heart failure who received CRT devices with defibrillator and 20 age-matched, healthy control subjects. At baseline and 3 months and 6 months after CRT, we assessed...
